John started for the 1966 Chicago White Sox and the 1989 New York Yankees on Opening Day. The Indians signed John, then 18, in 1961, based on his curveball. After three minor league seasons, he was called up in 1963. He spent two seasons with the Indians before being dealt with the White Sox. He made his major league debut in 1965 and started for Chicago in 1966.

On his first All-Star squad, he finished fifth in the American League (AL) with a 1.98 ERA, however, he missed the season due to a dispute with Dick McAuliffe. When Johnny Sain tried to get John to throw more sliders, he posted his worst ERA since 1964 and was traded to the Dodgers for Dick Allen. He led the NL in winning percentage for two seasons after winning 11 games with the Dodgers in 1972. In a game against the Montreal Expos in 1974, he tore his ulnar collateral ligament (UCL). Later that year, John had ligament replacement surgery with Dodger Dr. Frank Jobe. He missed the 1975 season while recovering from surgery, but he was the first pitcher to do so.

In 2012, one in every seven MLB pitchers had Tommy John surgery at some point in their careers. John had a 10–10 years after surgery. John went 20–7 with a 2.78 ERA in 1977, helping the Dodgers win the NL West and reach the World Series. He led the Dodgers to the 1978 World Series with a 17–10 record before signing with the Yankees. John had 20-win seasons with the Yankees in 1979 and 1980 and was an All-Star in 1978, his final season with the Dodgers.

His only World Series appearances were against the Dodgers and Yankees, and he pitched for the losing club in all three. In 1982, he was dealt with the California Angels, where he made two playoff starts. He pitched for the Oakland Athletics for the second part of the season after being released in 1985. John’s career seemed gone when he went unsigned in 1986, but injuries to Yankee pitchers led to his re-signing. After Phil Niekro retired, John became the oldest player in baseball in 1988, winning 13 games for the Yankees.

Before retiring in 1989, he tied Deacon McGuire’s record of 26 seasons pitched. For the Modern Era Subcommittee of the Veterans Committee, John was on the ballot from 1995 to 2009. He never garnered more than 31.7 percent of the votes (75 percent is required for election). In the 1990s, he worked for the Minnesota Twins and the New York Yankees before managing the Bridgeport Bluefish from 2007 to 2009. Since retirement, he has worked as a motivational speaker and launched the “Let’s Do It” Foundation to prevent suicides.
